  • Driver's License or Photo ID

  • Current medications (bring bottles)

  • Any medical records or laboratory results

  • Your health insurance card

New Patients

Our new patients may download our new patient forms from the "Forms" section or by clicking the button below. Please fill out all forms and bring them to your scheduled appointment to expedite the process.

If you have health insurance that requires a referral or authorization from your family doctor, we will do our best to obtain it. However, in some circumstances, you will need to obtain a referral to our office yourself. If you have had any heart tests (such as stress tests, echocardiograms, Doppler studies, cardiac catheterizations, or stents) performed in the past, please try to obtain a copy of the results and bring them with you.

 

All co-payments and deductibles are due at check-in (before your visit with the doctor and/or before a scheduled office test is performed). If you have not met your deductible, this will be expected at the time of your visit. We accept checks and most credit cards. Please arrive 15 minutes prior to your scheduled appointment time.
