Cancellation Policy:

Regular Appointments: Please call us 24 hours before your scheduled appointment to cancel/reschedule. A $20 cancellation fee will be applied to office visits cancelled too late.

Stress Tests: We are required to purchase the drugs required for stress tests ahead of time, and these drugs are costly. Please call us at least 24 hours in advance to cancel your stress test or else a cancellation fee of $200 will be applied.

For all prescription refills, first call your pharmacy and ask them to send the refill request to our office at (863)676-6431 (fax). The pharmacy can also send an e-prescription refill request. Please allow a minimum of 24 hours processing time for all refills, and ensure the prescriptions you have will not run out on weekends or holidays.
Patient Instructions

Your co-pay and outstanding balances are due at the time of service. If you have a deductible, this will be expected at the time of service. Please note that we do accept Visa, MasterCard, American Express, Discover, and checks.

If your insurance company requires a referral or authorization, be sure to have this ahead of time. We do our best to obtain this, but you may need to obtain it for us. This includes authorization for any testing and pacemaker/ICD checks.
Your appointment will need to be rescheduled if authorization is not obtained.

It is your responsibility to contact our office with insurance changes including the effective dates.

Bring the following with you to EVERY appointment:
1. Current medications (bring bottles)
2. Insurance ID card. The receptionist will need to update insurance information at every visit.
3. Log of blood pressure (if any)
4. Updates of any new prescriptions, hospitalizations, procedures/surgeries, tests
